Cyllene Therapeutics’ Gene Therapy Receives FDA RMAT Designation for Neurogenic Detrusor Overactivity

The designation was based on early U.S. trial data in spinal cord injury patients, where this non-replicating HSV-1 gene therapy is being tested as a single-course treatment aimed at reducing bladder overactivity without impairing normal function.

Key Points

  • Cyllene Therapeutics has received FDA Regenerative Medicine Advanced Therapy designation for EG110A in neurogenic detrusor overactivity with urinary incontinence.
  • The designation was based on early data from an ongoing U.S. Phase 1b/2a trial in spinal cord injury patients.
  • EG110A is a non-replicating HSV-1 gene therapy designed to reduce bladder overactivity while preserving normal bladder function, with a Phase 2b/3 study planned for 2027.

Cyllene Therapeutics has received FDA Regenerative Medicine Advanced Therapy designation for EG110A, its lead gene therapy candidate for neurogenic detrusor overactivity (NDO) with urinary incontinence. The designation adds to the program’s Fast Track status, which the company received in October 2025.

EG110A is being studied as a single-course treatment for patients with bladder overactivity linked to neurological injury. The therapy uses a non-replicating HSV-1 vector and is designed to selectively silence CGRP+ sensory neurons involved in bladder muscle overactivity while preserving normal bladder function. Cyllene says the treatment is site-specific and non-systemic, aiming to provide durable symptom relief after one treatment course.

Clinical Development

The RMAT designation was based on initial results from an ongoing U.S. Phase 1b/2a study in patients with NDO associated with spinal cord injury. According to the company, the early data showed proof of concept, durable efficacy, and a favorable safety profile. The company expects to present interim results from the study at the International Continence Society annual meeting on October 7, 2026, in Maastricht.

“Receiving RMAT designation from the FDA for EG110A is a key milestone in the history of the company. It recognizes the critical need to provide better therapeutic solutions to patients affected by severe urinary incontinence as well as the encouraging initial clinical results we have achieved to date,” said Philippe Chambon, MD, PhD, Co-Founder and Chief Executive Officer of Cyllene Tx.

“The clinical data seen to date, demonstrating a marked reduction in urinary episodes, suggests treatment with EG110A could have a profound impact on the daily lives of patients. We look forward to working closely with the FDA as we move further into clinical development, including a planned Phase 2b/3 study with EG110A in neurogenic detrusor overactivity in 2027,” said Cornelia Haag-Molkenteller, MD and Chief Medical Officer at Cyllene Tx.

Why It Matters

NDO is a chronic bladder dysfunction caused by neurological injury or disease. It can lead to uncontrolled urinary incontinence, urinary tract infections, and risk of kidney damage. The condition is common in people living with spinal cord injury, multiple sclerosis, and Parkinson’s disease. The press release states that at least 2 million patients across the seven major markets are affected.

RMAT designation is intended to speed development and review of regenerative medicine therapies for serious conditions when early clinical evidence suggests the treatment may address an unmet need. For Cyllene, the designation should allow closer interaction with the FDA on trial design, endpoints, and potential paths toward accelerated approval as EG110A moves into later-stage development.
